Wondering how Support at Home works?
The Support at Home program is a government-funded program designed to provide elderly Australians with the care and support they need to continue living at home. There are eight levels of care, ranging from basic assistance to more comprehensive care, based on your individual needs.
These packages allow you to access a wide range of services, including personal care, domestic assistance, meal preparation, transport, clinical services and more.
How to start receiving Support at Home:
1. Contact My Aged Care
Call My Aged Care at 1800 200 422 or visit their website to request an ACAT assessment. You can also ask your GP or hospital to refer you.
2. Home Visit Assessment
The ACAT team will arrange a home visit to assess your physical, medical, and psychological needs. They will discuss your daily activities and determine which level of care and services is most suitable for you.
3. Approval and Waiting Period
Once approved, you’ll be placed on a national waiting list for a Support at Home Package. The wait time varies depending on the level of care you require. During this time, Real Caring can provide advice on alternative support options if needed.
4. Choose your Provider
Once your package becomes available, you can choose Real Caring as your preferred provider. We will guide you through the next steps and assign you a dedicated Care Partner to manage your in-home care and ensure your care plan is tailored to your individual needs.
